Join UnitingSA's Community Aged Care team! Collaborate, assess, plan, and guide in clients' homes. Drive individualised therapy, offer direction to the team and ensure safe, effective care for consumers. Make a difference in supporting clients to live independently at home for longer. An exciting opportunity awaits!Key Responsibilities
  • Collaborate with clients, families and caregivers to ensure a holistic approach to care
  • Conduct Comprehensive assessments to identify and address challenges faced by older adults in daily activities
  • Devise & develop personalised therapy plans to optimise independence, wellbeing and safety
  • Liaise with clients, families, & diverse teams.
  • Create concise treatment plans & recommendations.
  • Educate on preventive measures tailored to comprehension levels.
  • Provide evidence-based interventions for varied consumer groups.
  • Evaluate and draft to-scale diagrams for home modifications.
  • Prioritize consumer-centric care delivery.
  • Update clients on equipment/modification progress.
  • Exhibit excellent time and resource management.
  • Promptly flag and address incidents and risks.
  • Maintain detailed records, case notes & handover docs.
  • Produce pertinent clinical reports.
  • Conduct personal care safety assessments and aid care plan development.
  • Expedite referrals to healthcare professionals.
  • Advocate for and connect clients with valuable community resources.
Skills & Experience:
  • Minimum 2 years' community aged care experience required.
  • Proficient in diverse assessments: showers, kitchens, transfers, equipment.
  • Extensive expertise in complex assessments with diverse groups.
  • Skilled in addressing individual, structural, and environmental risks.
  • Proven record in crafting tailored treatment plans and recommendations.
  • Capable of delivering intricate OT interventions in the community.
  • Familiarity with Aged Care Quality Standards.
  • Excellent, sensitive communication with consumers and stakeholders.
  • Strong prioritisation skills based on urgency and risk levels.
  • Understanding of the Occupational Therapy Code of Ethics.
