You will be passionate about keeping our clients independently safe at home whilst at the same time be able to assist them to manage their chronic disease and pain. - Have full registration with the Australian Health Practitioner Regulation Agency (AHPRA).
- Preferably have at least 2 years physio work experience. Experience in community-based physiotherapy settings is desirable.
- Are a confident driver and have a registered, insured and roadworthy vehicle, and possess a valid NSW Driver's License.
- 3 days availability (can be shorter days) - but potential for more in future.
- Be tech savvy. Have strong administration and time management skills.
- Enjoy flexible and independently driven working hours.
- Be a team player who enjoys working autonomously and collaboratively within the broader Anglicare At Home team.
- Have strong communication and interpersonal skills to work effectively within a team and to build positive relationships with clients and their families.
- Proven experience in aged care or a related field is preferred but not mandatory.
- Delivering evidence-based clinical care in the community through a range of programs and funding streams including but not limited to Home Care Packages, Commonwealth Home Support and Short Term Restorative Programs.
- Assessing, developing and implementing personalized physiotherapy plans/treatments to achieve developed goals with the aim of enhancing client independence, mobility and physical health. This may be delivered within client homes or in a class setting.
- Regularly reviewing and progressing plans to meet these goals.
- Organising other resources to assist with achieving these goals, which may include handing over and training/mentoring physiotherapy assistants.
- Collaborating with both internal and external health professionals, which include Physiotherapists, Occupational therapists, Registered Nurses and Care Advisors to provide holistic care and support for our clients..
