Together we can do more.Clinically guide a specialist Social Work service to patients, clients, consumers and their family/ carers to ensure the provision of optimal health outcomes consistent with ISLHD policies, procedures, standards, relevant legislation, and the NSW Health Code of Conduct.This position provides a high-quality specialist Social Work service for Homeless Health. The position is responsible for providing a high level of Social Work service to assist people experiencing homelessness to secure and/or maintain stable accommodation, through improved care planning and service coordination within health and between health and the broader community sector and the appropriate use of limited brokerage funds.The position will seek to strengthen partnerships and collaboration between health and the community sector and work closely with the NGO responsible for administering brokerage funds.
