Experienced Community Physiotherapist required for six month fixed term contract in sunny Darwin. The role is to start ASAP, ideally July or early August. This role sits within the Aged Care Unit, which consists of the Aged Care Assessment Service, Short Term Restorative Care and Transition Care, Community Frailty Service and Memory Service and Psychogeriatric Services. The focus of this role will be:

Community Transitional Care Program- up to 10 clients on a 12 week reablement program – if the role is a blend of Therapy and Coordination, then the caseload would be lower.
To coordinate and provide an opportunity for clients to reach restorative goals by improving physical, psychosocial and or cognitive functioning. This program aims to reduce the risk of hospital readmission and premature entry into residential aged care by utilising a multi-disciplinary team approach. This position is part of the Aged Care Unit which provide a number of services designed to meet the needs of our ageing population delivering a person centred, wellness and reablement approach.
You must have experience in TCP, aged care and/or community. Minimum three years relevant experience required.
July/early August start date.
Potential for 12 weeks of accommodation to be made available.
Salary of $90,204 to $107,554 per annum.
No travel assistance.
