The Connections Program works within an assertive linkage model that aims to link patients with relevant health and welfare service providers pre and post release.The Connections Transition Coordinator will utilise a broad array of contacts, both in the correctional and community environments to assertively link patients into services appropriate to their individual needs.Salary & BenefitsYour salary of $112,396 (Year 1) - $116,038 (Year 2) for Social Worker, Counsellor and Occupational Therapist classifications or $77,358 (Year 1) to $113,226 (Year 9) for Psychologists + super and leave loading is just the beginning.
