Find out more and .Your OpportunityMission Australia's Psychosocial Services in the Northern Territory help individuals with mental illness transition to the NDIS or provide support if they're ineligible. Following a recovery framework, we assist in achieving non-clinical goals for housing, finances, daily living, work, and overall well-being.This unique Case Manager opportunity will see you connect with people living in identified remote communities who are living with complex mental illness. Case management is provided in person and via tele-health so you will need to have the flexibility to travel regularly to Pupunya, Haast Bluff and Mount Liebig.Your key responsibilities will be to:Review referrals and evaluate eligibilityEngage with participants in a culturally appropriate manner, fostering connection and trustDevelop strong relationships with key stakeholdersProvide effective and recovery based approach case managementCreate and maintain up to date individualised case management files and recovery plansComplete a range of internal and external reportsAttend local community meetings, forums and community awareness events and conferences as requiredRequirements for SuccessMinimum Certificate IV qualifications in Mental Health or equivalent with previous experience working in the mental health sectorStrong understanding of Aboriginal and Torres Strait Islander cultures and communities – previous experience working within remote or regional groupsAbility to travel to remote communities on a regular basis, involving overnight staysA sound understanding of the National Disability Insurance Scheme (NDIS) access processA demonstrated person-centered, recovery-focused, strength-based approach to participants’ care, based on a sound understanding of mental health and illnessHighly developed verbal and written communication skillsExperience in developing and/or facilitating one-on-one and group activitiesCurrent unrestricted Australian drivers’ licenceA valid NT Working with Children Check (OCHRE Card)We welcome and encourage applications from Aboriginal and Torres Strait IslandersBefore starting work with us, you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.Our cultureOur innovative and inclusive culture will ensure you feel valued at Mission Australia as you help Australians in need and develop in your career.We’re committed to the safety and wellbeing of each other and the people we serve.We recognise and respect diversity.
