- Salary $91,785.00 - $106,330.00 p.a. (Pro Rata) AHP2| Part Time 22.5 hours per week - Contract Until January 2026
- Supporting triaging referrals
- Providing consultancy service to other staff and agencies re provision of health services for older people on a residential caged care pathway
- Working collaboratively within a multidisciplinary team to continually improve, implement and evaluate a holistic CAPS program which is responsive to identified community needs
- Supporting care co-ordination for clients on the SALHN CAPS Program through regular contact with service providers and the acute and subacute sector
- Demonstrated skills in case management/care coordination and the ability to make complex clinical decisions including sourcing and coordinating services to meet clinical and community care needs
- Demonstrated experience working within a multi-disciplinary team with contribution to the development and attainment of team goals
- Demonstrated sound clinical experience with older people in acute and/or community settings
- A degree in Social Work (eligibility for membership of the AASW)
- Proven experience in delivering high quality and safe care consistent with the National Safety and Quality Health Care Service Standards
