We are looking for a talented Registered Nurse to join our Remedy family. Working as part of our multidisciplinary healthcare team, you will coordinate and support the delivery of home-based health care to clients across Australia.This great part-time opportunity will be worked remotely on Saturdays and Sundays from 10am - 2pm on a 12 month fixed term contract.The day to day
- Coordinating and managing acute Hospital Care at Home programs for clients discharging early from hospital to home-based care
- Supporting and communicating with clients remotely to achieve their personal and clinical health goals within their home environment
- Implementing, monitoring and modifying in-home care plans to meet client needs
- Supporting scheduling activities
- Loading new cases into the system
- Performing ad-hoc administrative tasks as needed
- Managing clinical case escalations and variations to expected clinical progress
- Managing relationships with multiple stakeholders including hospital referrers, surgeons/specialists, service providers and funders
- Qualification as a Registered Nurse with current AHPRA registration
- Experience working in a multidisciplinary collaborative team
- Excellent knowledge of hospital discharge planning
- Tech savvy - this role requires working across multiple systems
- Ability to work in a dynamic environment with tight timeframes and competing priorities
- Excellent clinical reasoning and problem-solving skills
- Excellent verbal and written communications
- Strong administration and time management skills
