This full-time role is all about empowering individuals to live their best lives-supporting them to achieve their goals and participate fully in everyday activities.You'll work with a diverse client base, providing services through a variety of programs, including:
- Private Services - Tailored support for individuals with self-identified needs
- Rehab in the Home - Post-orthopaedic surgery recovery
- Hospital in the Home - Support following general hospital stays
- Home Care Packages - Helping clients maintain independence at home
You're AHPRA-registered, passionate about person-centred care, and thrive working independently in a community setting. You'll be mobile across Geelong, so a reliable vehicle is essential (we offer kilometre reimbursement).
