
Home Care Coordinator

Your day-to-day will include:
- Managing incoming referrals and conducting client assessments
- Developing individualised care plans tailored to client goals and needs
- Coordinating staff schedules and matching carers with clients
- Monitoring service delivery to maintain quality and client satisfaction
- Maintaining accurate client records and complying with relevant policies and procedures
- Responding promptly to incidents, feedback, or concerns
- On-call roster - allowances and call out levy - additional $
- Minimum Certificate III in Aged Care, Individual Support or equivalent (essential)
- Current First Aid Certificate
- Valid Australian Driver’s Licence and access to a reliable vehicle
- Current Police Check
- NDIS Worker Screening Check
- Experience home care packages
- Excellent interpersonal skills, with the ability to build strong relationships with clients, families, and staff
- Sound understanding of the Aged Care Quality Standards and person-centred care principles
- Confidence using care management systems and Microsoft Office
- Strong time management and multitasking ability
