- Conducting comprehensive bio-psycho-social assessments.
- Undertaking clinical mental state and risk assessments.
- Facilitating escalations of care when appropriate (e.g., to CATT, PARC, inpatient units).
- Participating in weekly multidisciplinary clinical reviews.
- Delivering brief therapeutic interventions such as psychoeducation, coping strategies, or single-session family work.
- Contributing to the development of collaborative safety and support plans.
- Maintaining accurate and timely clinical documentation.
- Tertiary qualifications in Social Work and eligibility for AASW membership.
- Experience in clinical mental health assessment and risk management.
- Strong communication and interpersonal skills.
- Ability to work effectively within a multidisciplinary team and in partnership with external providers like BPA.
- A client-centred approach with a focus on collaboration, engagement, and holistic care planning.
- Commitment to maintaining high standards of clinical documentation and care coordination.
- A valid drivers licence.
