Employment Status: Fixed Term Part Time (until June 2026)Closing date: 14/09/2025The Residential in Reach (RIR) is a nurse led program that provides specialist assessment, co-ordination, care provision, and treatment for residents within aged care facilities. The service provides an alternative to the Emergency Department for the management of acute conditions and aged related issues. The program provides clinical intervention that is short term and person centred.This project officer role will focus on supporting the Loddon Mallee Health Network Residential In Reach teams to expand their service delivery into additional geographical areas to maximise opportunities for residential aged care residents to receive care where they live and increasing opportunities to provide post hospitalisation support for those aged care residents at risk of unplanned readmission.This role will be responsible for overseeing data collection from each program, reviewing agreed implementation plan KPIs and developing relevant reports.
