Signature Care presents an exciting opportunity to shape the service delivery of aged care into the future during a period of significant change.We are currently seeking an experienced and qualified Clinical Care Coordinator to work Permanent Part Time 8 shifts per fortnight, to work closely with our Clinical Manager and Residential Manager to develop our stunning 180-bed aged care residence based in Tinana.The Clinical Care Coordinator (CCC) will provide day to day clinical leadership to the team, foster a culture of clinical excellence and oversee the clinical care of our residents to ensure that a high standard of care is maintained.Skills and requirements for this role:

Ideally at least 2+ years experience in a Clinical Care Coordinator or similar role in the aged care or acute sector;
RN qualification with current AHPRA registration;
Excellent and up to date clinical knowledge and assessment skills;
Experience in staff supervision and development;
Strong communication and negotiation skills;
A working knowledge of funding and accreditation processes;
Knowledge of Care Planning.
