The Division of Aged, General Medicine and Subacute Services (AGMS) has an exciting opportunity for an enthusiastic and motivated registered nurse ready to step into a dynamic leadership role.Our Bed-Based Transitional Care Program ward is a 30-bed ward that is dedicated to delivering exceptional care in a dynamic and nurturing environment, focused on enhancing the recovery and independence of older people post-hospitalisation and supporting their journey back into the community or on to Residental Aged Care.We're looking for a dynamic nurse leader with proven experience and motivation with a passion and strong commitment for caring for the older person to lead our team. This is an opportunity to inspire and manage a dedicated group of healthcare professionals while fostering a culture of continuous improvement, teamwork, and excellence in care.Step into a role where you'll help shape the transition of the Strengthened Aged Care Standards, collaborate across Western Health committees, and be empowered to pursue leadership development tailored to your career goals.Key responsibilities:You will play a pivotal role in implementing and ensuring compliance with the Strengthened Aged Care Standards, actively shaping the future of aged care at Western Health. This includes:

Leading the integration of the Aged Care Quality Standards into daily practice
Collaborating across Western Health committees to ensure alignment with national aged care reforms
Championing initiatives that promote safety, dignity, and quality of life for older people
Driving continuous improvement through evidence-based practice and audit processes
Provide effective leadership and operational management of the ward team
Ensure delivery of high-quality care aligned with the principles of the Aged Care Quality Standards
