Provision of care coordination for older people who present with complex mental health issues across 2 sites. This may include physical co-morbidities, psycho-social challenges and issues of addiction of substance use. The successful applicant will be required to provide education and guidance to internal and external partners including Residential Aged Care Facility staff.People of Aboriginal and/or Torres Strait Islander background are encouraged to apply.Candidates will need to meet the following criteria:
- Registered Nurse who has a minimum of 5 years full time post graduate experience in a mental health specialty. In addition, the applicant must have an approved post graduate qualification within the speciality of mental health drug and alcohol or other such qualifications or experience deemed appropriate.
- Demonstrated experience working with consumers and their carer/s presenting with complex mental health needs and/or behaviours of concern and demonstrated ability to assess, plan, implement and evaluate interventions and care plans.
