- Demonstrated previous experience in client assessment and Case Management in a community setting, preferably in aged care
- Sound understanding of all relevant legislation as well as the Single Funder Model being introduced from July 2025.
- Demonstrated ability to work collaboratively within a small team environment
- Demonstrated knowledge of, and ability to access a range of community resources and professional services for clients
- Demonstrated ability to work effectively with a culturally diverse group
- Demonstrated ability to work through challenges in systematic and creative ways
- Demonstrated ability to work autonomously with a high degree of initiative and attention to detail
- Demonstrated ability and confidence to actively promote services to build client base is highly desirable
- Excellent communication skills, both written and verbal
- Proficient in the use of computer software: Word, Excel, Internet, email and client data bases
- Demonstrated commitment to the 'Purpose and Principles' of Annecto
- A current valid Australian driver's license.
- Experience using Client Management Systems such as Techone, Alayacare
- An understanding of the ageing process and its cultural impact;
- An ability to speak a community language/s.
- You must have evidence of your eligibility to work in Australia.
