Case Manager - KempseyWe are looking for another Case Manager! This is an exciting opportunity to work with an independent, not for profit organisation located in Mid North Coast - NSW working with a small, friendly and client focused team. As well as Case Management with our Aged Care Funded clients you will also be actively involved in service promotion, providing advice on access to community based aged care to community members and working closely with our team of wonderful direct Support team members. As well as Home Care Packages, we also provide Short Term Restorative Care, CHSP Other Food Services, Veteran Home Care and brokerage to other agencies and private fee for service.   • Demonstrated previous experience in client assessment and Case Management in a community setting, preferably in aged care
  • Sound understanding of all relevant legislation as well as the Single Funder Model being introduced from July 2025.
  • Demonstrated ability to work collaboratively within a small team environment
  • Demonstrated knowledge of, and ability to access a range of community resources and professional services for clients
  • Demonstrated ability to work effectively with a culturally diverse group
  • Demonstrated ability to work through challenges in systematic and creative ways
  • Demonstrated ability to work autonomously with a high degree of initiative and attention to detail
  • Demonstrated ability and confidence to actively promote services to build client base is highly desirable
  • Excellent communication skills, both written and verbal
  • Proficient in the use of computer software: Word, Excel, Internet, email and client data bases
  • Demonstrated commitment to the 'Purpose and Principles' of Annecto
  • A current valid Australian driver's license.
What would be great to have?
  • Experience using Client Management Systems such as Techone, Alayacare
  • An understanding of the ageing process and its cultural impact;
  • An ability to speak a community language/s.
