Kyneton Community Mental Health Team have an opportunity for someone to join their team in an Fixed Term Part Time, this role is within the Macedon Ranges and the successful applicant will be working Monday to Friday 8:30am - 5:00pm.You will provide intake coordination, assessment, and clinical treatment-including crisis support and case management-for patients, families, and carers; develop and monitor Individual Service Plans in collaboration with all stakeholders; engage with community services for ongoing support and discharge planning; and participate in education, consultation, and multidisciplinary team activities.Working with Bendigo HealthBendigo Health are a leader in regional health care, combining outstanding services with a strong education, training and personal development culture.Created by our community in the 1850s, we have a long, proud history of delivering excellent care to our community.Our organisation is diverse, welcoming and committed to delivering our vision of Excellent Care.
