Access theThe roleLead and manage the Maternity to Child and Family Health Transition of Care (MAtCH) project within the Health Contact Centre (HCC) as an initiative under the Putting Queensland Kids First program being led by Children's Health Queensland.The project was established in February 2025 to progress a pilot to provide families with an early clinical telehealth consultation after discharge from maternity services. This is a unique opportunity to collaborate with the West Moreton Hospital and Health Service on this pilot to support transition from maternity care to child health care services within the community. It will involve service planning and design, information communication technology solutions and organisational change to test the service model for its potential roll out to other sites.Key responsibilities:
- Lead effective and efficient delivery of the project managing scope, time, cost, procurement, quality, risk, human resource and communication management in line with endorsed project management frameworks.
- Manage project resources, expenses and capital costs against the project forecasts and report on all project elements as required.
- Enable informed decision making on project issues by developing timely and relevant written submissions, briefs, presentations and information papers for project stakeholders.
- High level of project management skills, including the design of large scale and complex projects with the ability to achieve successful project outcomes including leading, managing, and directing multi-disciplinary project team/s.
- Excellent leadership and people management capability with the proven ability to deliver projects in collaboration with health service stakeholders.
- Understanding of the whole health system, including the complex interrelationships within the state and federal governments, non-government and private sectors, Primary Health Networks, Hospital and Health Services, Aboriginal and Torres Strait Islander Community Controlled Health Organisations, and primary and community care providers and peak bodies.
- Ability to enable informed decision making on project requirements and issues by developing timely and relevant written submissions, briefs, presentations and information papers for project stakeholders.
- 12.75% superannuation
