Access theAbout the opportunityThis position provides nursing resource, education and research accountabilities and strategic management to facilitate the delivery of high quality, best practice care to clients of the Transition Care Service.About the opportunityThe Townsville Transition Care Program operates as a hub and spoke model in Townsville (hub) and Ingham, Ayr, Home Hill, Charters Towers and Mt Isa. Service provision aligns with Schedule A of the Commonwealth Transition Care Programme Guidelines. Services are referral based and delivered in-home by sub brokered Non-Government Organisations (NGO's)Elder care operates as an aged care placement consultative service to the TUH inpatient teams and wards. Patients are identified via SNAP, ACAT referrals for permanent care approvals and/or paper referral (via activation form). - Salary range: $5081.10 to $5516.60 p.f. (NG7)

- Qualification: Bachelor of Nursing or equivalent certification relevant to the position is mandatory.
- Highly Desirable: Possession of a relevant postgraduate qualification in an appropriate field is highly desirable. Such experience may be discipline specific or have a service area focus.
- Aged Care Pre-Employment Screening: Applicants are advised this position works in an identified Aged Care Service as per the Aged Care Act 1997 and therefore requires employees and volunteers to have a current National Police Certificate. Queensland Health will facilitate applicants obtaining the above National Police Certificate.
- Drivers Licence: This position requires the incumbent to operate a class C motor vehicle and an appropriate license endorsement to operate this vehicle is required. Proof of this endorsement must be provided prior to the commencement of duty.
