- Rotating roster including night duty
- Co-ordination of client care needs, including assessment, case management and developing individualised care plan
- Assisting the clinical management team with ACFI documentation & comprehensive assessments
- Providing support & supervision of Care Assistants
- Liaising with clients and their significate others, and members of a multidisciplinary team to achieve positive client outcomes.
- What you will need to succeed:
2+ years of experience in a similar role is desirable
Able to obtain a National Police Certificate and / or NDIS Yellow Card
Great clinical Skills
Demonstrated up-to-date knowledge and skills in contemporary Aged Care Services
Excellent written & verbal communication skills
A genuine commitment to lead, coach and develop other care staff and a passion for making a differenceThe Location
