In this role you will be responsible for delivering holistic, person-centred and trauma-informed care to people with complex mental health needs. You will be working closely with their families and carers. The focus is on adults with acute, complex or high needs and who may be difficult to engage in effective treatment and support.
Identified outcomes are:
- Successful therapeutic engagement with adults experiencing, or at risk of deterioration of, a severe mental illness.
- Better experience of care through integrated and coordinated service models
- Prevention of unnecessary hospitalisations through timely access to appropriate and targeted clinical care
- Active use of a multiagency care plan for adults with, or at risk of, severe and complex mental illness, to help link providers across multiple services involved in their care
We work closely with many community organisations including Medicare Mental Health, Grand Pacific Health, Headspace and Wellways, to ensure people receive the right care for their needs. At a district level we have access to clinical leaders for MH, AOD and other specialty services such as Aboriginal Mental Health, eating disorders and Perinatal Infant Mental Health.What MHDA offer:As our employee you will have access to education and training to support your professional development. - Have an in-depth understanding and ability to provide Youth Mental Health services in a curious and empathetic way
- Demonstrate a high level of professionalism with ability to work independently and as a part of a multidisciplinary team
- Have sound communication skills, both written and verbal
- Be able to manage and prioritise a diverse workload
- Hold a relevant Australian qualification as a Counsellor, Psychologist, Social Worker or Registered Nurse, or must hold a minimum of a bachelor's degree in a relevant field e.g. community welfare, or other qualification deemed equivalent by the employer.
