In this role, you will provide rapid and responsive support to patients with complex medical and social needs, helping to reduce hospital admissions and re-presentations. You'll work with individuals facing challenges such as NDIS support needs, acute carer stress, breakdowns in support systems, reduced capacity for daily living, and frequent presentations to hospital.Based within the Social Work Department, you will respond to referrals from the Emergency Department and other hospital services, playing a key role in ensuring patients are connected to the right supports. You'll enjoy being part of a multidisciplinary team, bringing the social work perspective to collaborative care, and helping patients transition smoothly through the health system and beyond discharge.To support your success, you'll receive a comprehensive orientation program, regular clinical supervision, peer support, and access to ongoing professional development and training opportunities tailored to your career goals.About you:
Our successful candidate will possess the following:
- Demonstrated advanced clinical experience (minimum 3 years full time) in crisis intervention including trauma, counselling, case management for patients/families with illness adjustment, domestic violence, child protection, end of life care and bereavement support.
- Demonstrated ability to independently problem-solve and stay within professional and ethical boundaries when working with complex/pressured situations as part of a multi-disciplinary team.
- Social Work degree and eligibility for the Australian Association of Social Workers (AASW).
