Through our Consumer Directed Care model, you will work directly with consumers to help them maintain independence and make choices about their care.Key Responsibilities:
Provide personal care, cleaning, meal preparation, and transport
Work with consumers, families, and health professionals to create and maintain personalised care plans
Offer emotional and social support to improve well-being and independence
Support consumers to meet their goals and preferences in a collaborative manner
What we're looking for:
Compassionate, empathetic, and flexible approach to care
Ability to work autonomously and as part of a team
Strong communication and interpersonal skills
Previous experience in aged care, disability, or community services is preferred
