- Work effectively with clients, their families, and other stakeholders to deliver high-quality, person-centred care.
- Attend and participate in case conferences, employee, and stakeholder meetings, documenting outcomes and complete actions within specified timeframes.
- Maintain accurate and timely records in compliance with policies and procedures.
- Develop care plans and agreements in collaboration with clients and stakeholders.
- Prepare support letters and recommendations for clients for governing body reviews.
- Monitor and evaluate service delivery through quality improvement measures.
- Manage client issues and respond to feedback effectively.
- Provide holistic and individualised case management within a person-centred framework.
- Tertiary qualification in Nursing, Allied Health, Disability or a related field preferred. Registration as an EN would be preferred.
- Experience in the disability or health sector, with an understanding of the challenges faced by people with disabilities and their families.
- Sound knowledge of the NDIS scheme, including standards, policies, and procedures.
