We're looking for a dedicated Aboriginal Case Manager for our Home Care Package (HCP) Program. This is your opportunity to use your skills and expertise to deliver personalized care that supports individuals in their chosen environment, helping them maintain their independence and live fulfilling lives.About the Role:As an Occupational Therapist, you will bring clinical experience, developing generalist and/or specialist knowledge, and professional competence to plan, implement, and evaluate a comprehensive and integrated range of services that meet the needs of the local community.You will work under reduced clinical direction and contribute to workforce development through the clinical supervision of less experienced allied health professionals, allied health assistants, and students.Collaboration will be central to your role, working as part of a multi-professional team alongside colleagues from both health and other sectors. You will deliver services using a mix of preventative, early intervention, treatment, therapy, and evaluation approaches, ensuring holistic and person-centred care.What We're Looking For:
  • Must hold a recognised qualification within the Occupational Therapy profession and be eligible for practicing membership of the relevant Professional Association. For those professions requiring Registration, all requirements to obtain and maintain current registration must be fulfilled. For self-regulated professions it is desirable to participate in the professional association accredited continuing professional development program.
  • Apply professional expertise to build generalist/specialist knowledge, clinical competence, and experience in complex practice settings.
  • Exercise professional judgment in adapting and selecting appropriate methods, procedures, and techniques.
  • Deliver a broad range of clinical and consultative services across varied settings, including individual care, group work, and health promotion.
  • Manage and prioritise personal workload while supporting others in developing workload plans and allocating team resources.
