πŸ’° Competitive Salary + Salary packagingheadspace Penrith, Hawkesbury and Mount Druitt, run by Uniting, aims to reduce the burden of disease in young people aged 12-25 years caused by mental health and related substance use disorders through early identification and effective, evidence-based intervention.Role Overview:Join headspace, Hawkesbury as a Physical Health Nurse leading a Capacity Boost Project to support young people aged 12–25. This role focuses on improving timely access to physical health care through health screenings, assessments, education, and GP collaboration. You'll help set up an on-site clinic, build care pathways with GPs, and work closely with a multidisciplinary team to improve youth wellbeing.As the Physical Health Nurse, your role specifically will include but not limited to:
  • Lead and establish a physical health clinic at headspace Hawkesbury
  • Collaborate with headspace teams and GPs to increase physical health access
  • Provide assessments, health education, and chronic care planning
  • Contribute to pathways between headspace sites and local health providers
Essential Criteria:You will have excellent written and verbal communication skills, be organised, systematic, thorough, accurate and disciplined.
  • Registered Nurse (AHPRA) + valid NSW Driver’s Licence
  • 3+ years' experience, including work with GPs and young people
  • Skilled in physical health promotion/resource development
Even better:
  • Experience working with discrete communities such as young people who identify as Aboriginal and/or Torres Strait Islander, CALD background and LBGTQIA+
  • Experience working in the Western Sydney and Nepean Blue Mountains communities
