- Conduct thorough Occupational Therapy assessments on admission and as required to create tailored therapy plans
- Perform Home Safety assessments and recommend home modifications to support independent living
- Prescribe assistive technology and equipment to enable clients to live comfortably and confidently at home
- Collaborate with clients, families, stakeholders, and Allied Health Professionals to address therapy needs
- Deliver evidence-based therapy to help clients maintain or improve their daily living abilities
- Work alongside the Allied Health Team to support wellness and reablement program in both residential and community care environments
- Demonstrated experience applying effective treatment and evaluation techniques in an Occupational Therapy setting
- Excellent interpersonal skills with the ability to work collaboratively across multidisciplinary teams
- Strong communication and documentation skills, with a client-centered approach to care
- Ability to manage a varied caseload and adapt to changing priorities
- A passion for supporting older adults to achieve their highest level of independence
- Bachelor’s degree in Occupational Therapy with current AHPRA registration – essential
- Experience in community or rehabilitation settings for older adults – highly regarded
- Current South Australian driver's licence – required for travel across LHG sites
- NDIS Worker Screening Clearance – required (NDIS risk-assessed role)
- Willingness to undertake a pre-employment medical assessment
