  • Are you passionate about supporting older people to live independently and safely in their communities?
  • Do you enjoy working collaboratively within a multidisciplinary team to deliver holistic, person-centred care?
  • Can you apply clinical reasoning and evidence-based approaches to support recovery and independence post-hospital discharge?

  • Assess, plan, and implement occupational therapy interventions tailored to clients' needs within the Transition Care Program, promoting independence and safe community living
  • Collaborate closely with health professionals, community service providers, clients, and their families to ensure integrated, person-centred care
  • Support and mentor less experienced staff, allied health assistants, and students (AHP2 role) or contribute under supervision (AHP1 role)
  • Assist in evaluating client progress and contribute to continuous quality improvement initiatives within the service
