Provide client-centred services such as home assessments, equipment prescription, and home modifications
Support client's post-hospital discharge to enhance safety and reduce the risk of readmission
Additionally for residential aged care clients you will develop and run group and individual programs

Prepare clear, practical reports that directly support care delivery-without the burden of traditional bureaucratic documentation

👤 About You

AHPRA registered (Full or Limited - Remote Level)
New grads welcome; experience in community or aged care is a plus
Skilled in promoting health and wellbeing through client-centred care
Comfortable working independently and travelling across Hobart (vehicle required; travel reimbursement provided)
