We're seeking a flexible and empathetic Community Support Worker to join Eyre and Far North Local Health Network, providing vital aged care, disability, and support services under the Consumer Directed Care model.In this role, you'll assist individuals who are aged, frail, or disabled-whether living at home or in a Residential Aged Care facility-with personal care, cleaning, meal preparation, transport, and social support. You'll work autonomously and collaboratively with consumers, their families, and health professionals to deliver quality care that respects consumer preferences, including their control over shifts, duties, and service providers.Eyre & Far North LHN values ongoing education and professional development, encouraging you to grow your skills while delivering responsive, high-quality care to our community.What We're Looking For:
Current drivers licence and willingness to drive
Current police clearances (refer police check clause)
