Make a meaningful difference with BlueCareWe’re looking for someone who wants to build meaningful relationships and make a difference in the lives of others -through small moments that leave big impacts.About the role:As a Senior Personal Carer you’ll conduct initial client intake and screening assessments and developing and reviewing individualised care plans with a client-centred approach while providing quality care based on clients’ needs.
  • Conduct thorough initial screening assessments, developing care plans for personal self-care, general house cleaning, social support, respite, transport and shopping assistance.
  • Management of client information in CRM & CMS ensuring all details and case notes are kept up to date
  • Engage in ongoing care discussions, planning, assessment, reviews, and evaluating goals and outcomes in consultation with the care team and as required.
  • Spending your time on the road completing scheduled visits, interacting one-on-one with clients within their homes supporting with daily activities
  • Providing care in accordance with their individualised Care Plan
  • Supporting with community based recreational and social activities
  • Transportation and assistance with shopping and appointments.

  • Certificate III Aged Care, Individual Support or an equivalent qualification - Essential
  • Current Australian Driver’s Licence (Green Provisional minimum) - Essential
  • Roadworthy & insured (Third Party Property minimum) vehicle to use for work- Essential
  • Experience with technology incl. smartphones, tablets, laptops etc
  • Confidence to satisfy the relevant probity checks required by legislation or policies
