Moree Community Health is seeking a passionate and motivated Diabetes Educator and Cardiac Rehabilitation Clinical Nurse Specialist Grade 2 (CNS2) to join our dedicated multidisciplinary team.About the Role
As a CNS2, you will:
- Provide individualised, case-managed care for people living with diabetes and those requiring cardiac rehabilitation.
- Deliver both community and acute support across Moree Health Services.
- Participate in outreach services supporting Warialda, Bingara, Mungindi, Boggabilla, and Toomelah.
- Work collaboratively as part of the Medical Outreach Indigenous Chronic Disease (MOICD) Program, alongside a Dietitian and Aboriginal Health Worker.
- Act as a clinical leader, contributing to high-quality, patient-centred care within your specialty area.

You bring:
- Postgraduate qualifications in Diabetes Management and/or Chronic Care (or commitment to complete within 12 months).
- At least 3 years' postgraduate experience (consideration will be given to those with less, subject to qualifications and progression pathway).
- Strong communication and collaboration skills with a commitment to patient-centred care.
