Advising and negotiating with referrers, and escalating any bed blocksWork in a dynamic team leading our approach to the provision of quality care to community clients of the Palliative Care ServiceProvide evidence-based nursing care to people of all ages and cultural groups, including individuals, families and communitiesTake an extended leadership role in the coordination of nursing and health care, providing expert nursing assessment and triage to facilitate optimal health outcomes.
