The opportunityAre you a Registered Nurse with a passion for supporting vulnerable and disadvantaged people in the community to address their health care needs? We are looking for someone to join our team at the St Patrick's Nursing clinic in Fremantle Working Monday and Wednesday 8.30am - 1.30pm.Our Non-for-profit specialist service supports vulnerable people in the community who are homeless, or at risk of homelessness. The services include (but are not limited to) clinical assessments, chronic disease management, wound care, diabetic care and monitoring, health education, promotion, and navigation, as well as referral to other services as required.Key ResponsibilitiesYou will be an experienced, multi-skilled Registered Nurse with the ability to provide primary health care including wound care, chronic disease management alongside health promotion, and system navigation.About you

3 years post-registration experience, with a broad experience in general nursing.
Experience in primary care
A strong desire and ability to work with individuals with complex unmet needs, those experiencing homelessness.
Skills to adapt to changing circumstances as the service develops and grows.
A current registration with Australian Health Practitioner Regulation Agency.
