Clinician & Clinical Care/Case Management - Social Worker

- You will be responsible for the provision of high quality and evidence based clinical services including intake assessment, referrals and complex case management.
- You will be responsible for coordinating a caseload across a wide range of individuals and their families.
- You will be maintaining clinical records, and report writing as required.
- You will be collating and analysing client data.
- You will be liaising with both internal and external stakeholders to ensure the appropriate level of care is assigned.
- You will be getting involved in community development projects as required.
- You will have experience in a similar clinical role
- You will have a passion for clinical psychology and social work to support mental health initiatives.
- You will have extensive case management experience within a mental health or social/community services organisation
- EITHER Degree Qualified + Current registration with the Australian Health Practitioners Registration Agency (AHPRA) OR
- OR Social Work Degree + Current membership with Australian Association of Social Workers (AASW) OR
- OR Masters Counselling Qualification + PACFA Registration as a Clinical Counsellor OR
- OR Masters Counselling Qualification + ACA at Level 3 or Level 4
- You will have Working with Children (WWC) and/or Vulnerable People Checks/Clearance ideally, or have it granted before commencing the role.
