Speciality - General Practice Pharmacist
Location - Bunberry
Start date - ASAP
Contract length - Permanent/Full Time
Pay rate - Dependant on Experience
Key responsibilities include:
Conducting medication reviews (e.g., MedsCheck, HMRs)
Supporting chronic disease management and medication adherence
Reconciling medications post-discharge and reducing polypharmacy risks
Providing evidence-based advice to prescribers on medication safety, cost-effectiveness, and guidelines
Educating patients on medication use and potential interactions
Participating in clinical audits, quality improvement initiatives, and health promotion activities
