And so do you.Join us as a Clinical Nurse - Resident Assessment and Documentation (RAD) and be there to care.A brighter future with Helping Hand, for older people and for you.A leader at our Copperhouse Court Residential Care Home, as a Clinical Nurse RAD you will work as a member of the care delivery team to ensure that residents’ health and care needs are appropriately assessed, and relevant documentation is undertaken for all residents, both on admission and when care needs change.This role will monitor and validate the AN-ACC funding and care minutes applicable to each resident and seek a review as the resident’s health needs change.You’ll also make an impact by:
- Ensuring the delivery of quality, person-centered care through assessment and care planning practices and inclusion of the resident or their substitute decision-maker where appropriate
- Actively participating in quality activities to ensure quality of clinical assessment and documentation is maintained and improvement opportunities are identified and acted upon
- Providing leadership, education, skills development, and day-to-day support of staff in all aspects of resident assessment and documentation
- Educating, mentoring, and supporting team members in relation to resident assessment, care planning and documentation and the delivery of care and services that support the AN-ACC classification
- A Bachelor of Nursing qualification
- A current AHPRA registration
- Demonstrated experience in and sound knowledge of the application of clinical assessment, evaluation, and care planning practices
- The ability to communicate with a diverse range of people from diverse cultural and religious backgrounds and age ranges both orally and in writing
- Demonstrated experience in a team leadership role including workload management and performance management
- Experience in preparing routine documentation related to care notes, preparation of reports and analytical and critical thinking skills
- The ability to operate smartphones and apps (such as Microsoft Outlook)
- Have a valid NDIS Worker Screening clearance or be willing to obtain one
- Post graduate nursing qualifications relevant to Aged Care
- Previous experience in a Clinical Nurse role or similar role
- Superior written and verbal communication skills including experience in:
- preparing reports
- analysing data; and
- providing recommendations.
