Our facilities and services are world class and our staff are dedicated, innovative and caring.About the roleThe SLHD Community Transition Aged Care Program (TACP) serves to transition aged care clients safely from hospital back into the community. The TACP Physiotherapist, in conjunction with the multidisciplinary TACP team, will work with clients to achieve their goals and ensure safe discharge from the program.The position is a community role, covering the Sydney Local Health District geographical area. TACP has its office base at Concord Hospital, but you may be asked to commence or finish work from other SLHD locations which include RPA, Canterbury and Balmain Hospitals.   • A Degree in Physiotherapy or equivalent, current registration with the Australian Health Practitioner Registration Agency (AHPRA) and a current unrestricted NSW Drivers licence (P2 acceptable).
  • A Postgraduate Physiotherapy experience, including aged care and/or rehabilitation and experience delivering Physiotherapy in the community setting.
