Access theAbout the roleThe purpose of this role is to deliver high level, evidenced based Occupational Therapy services and coordination of care for consumers of the Mental Health Intensive Discharge Team (MIND), their carers and relevant stakeholders, as part of an interprofessional team to expediate discharge from Metro North Mental Health Facilities.About youOur ideal candidate will be someone who can carry out the following key accountabilities in accordance with the Metro North Health's values:
- Apply high level specialised knowledge of physical, neurological, psychiatric, developmental, cognitive and sensory disabilities for patients with complex Psychosocial disability and challenging behaviours, including use of evidence-based management principles, across inpatient and community settings, to avoid unnecessary hospital admission and facilitate timely discharge.
- Use and model high level autonomous problem solving and critical thinking to resolve complex problems and facilitate independent clinical reasoning and professional judgement within MIND and in consultation with key stakeholders.
- Work collaboratively in an interprofessional model via application of high-level communication skills with other members of the MIND team.
