You’ll provide personalised guidance, link residents with care and wellbeing services, and foster a strong sense of belonging within the community.Your opportunityIn this full-time role, you’ll be a cornerstone in understanding and addressing the care needs of our residents while promoting health, wellbeing, and social connection.With care and professionalism, you will:Undertake health and wellbeing assessments for new and existing residentsBuild rapport and identify services that best meet residents’ needsSupport residents through the My Aged Care process, including referrals and advocacyWork closely with the Onsite Care team to meet health and social needsOrganise a calendar of wellbeing programs, groups and events that strengthen social connectionWhat you’ll bringYou’re compassionate, proactive, and motivated by a genuine desire to support older Australians to live well and independently.You will need:Strong interpersonal and communication skillsAbility to build trust and advocate for residents’ needsKnowledge of My Aged Care and community aged care services (desirable)Organisational skills to plan and coordinate events and programsA collaborative approach and commitment to person-centred careWhy Anglicare?At Anglicare, you’ll get the security, stability, and opportunities that come with a strong, well-established organisation that’s ready for the future.
