As the Case Management Team Leader, you will provide operational oversight and strategic direction to a team of case managers. You’ll ensure services are culturally safe, strengths-based, and responsive to the social and emotional well-being (SEWB) needs of Aboriginal and Torres Strait Islander clients. This role is pivotal in fostering a collaborative, high-performing team culture while maintaining service excellence and accountability.Key ResponsibilitiesLeadership & Mentoring
- Lead and support a team of case managers delivering non-clinical, culturally informed services.
- Provide cultural guidance and advocacy for clients navigating complex challenges.
- Mentor staff to achieve individual and team KPIs, promoting continuous improvement.
- Approve and monitor client case plans, review cycles, and data accuracy.
- Conduct performance reviews, identify skills gaps, and facilitate professional development.
- Use data analytics to inform decisions and escalate concerns when needed.
- Ensure compliance with internal policies and best practices through regular audits.
- Foster open communication and a supportive team environment.
- Facilitate peer consultations and complex case reviews.
- Apply conflict resolution strategies to manage complaints and feedback.
- Promote holistic support through appropriate referrals for high-needs clients.
- Respond promptly to internal and external communications.
- Maintain strict confidentiality and uphold privacy standards.
- Participate in staff meetings, training, and professional development activities.
- Proven experience in case management or social services leadership.
- Deep understanding of cultural safety and SEWB principles.
- Strong communication, conflict resolution, and team-building skills.
