Case Management Officer – First Nations Identified Role
Location: Brisbane, QLD
Employment Type: Full-time, Permanent
Salary: $81,000 + 15% SuperRole PurposeSupport and empower vulnerable, isolated, and at-risk individuals through a culturally responsive, client-centered, and strengths-based case management approach. This role is dedicated to First Nations people and aims to uphold self-determination, dignity, and holistic wellbeing for Aboriginal and Torres Strait Islander clients.Key ResponsibilitiesClient Engagement & Support
  • Deliver direct client services including intake, needs assessment, support planning, and coordinated referrals to community and legal services.
  • Build trusting relationships with clients to support informed decision-making and goal achievement.
  • Facilitate the development and review of Case Management Plans (CMPs) in partnership with clients.
Case Management
  • Manage an active caseload of up to 20 clients, each presenting with diverse and complex needs, vulnerabilities, and risk factors.
  • Provide ongoing support, advocacy, and coordination of services tailored to individual client circumstances.
Records & Data Management
  • Accurately document all client interactions, daily case notes, and service activities using the Client Management System (CMS).
  • Ensure all client records, consents, and authorities are current and securely maintained in line with privacy and confidentiality standards.
  • Complete required forms, checklists, and aggregate CMP data for monthly statistical reporting and evaluation.
Stakeholder Coordination
  • Liaise with internal legal teams and external social support services to ensure seamless and culturally safe referrals.
  • Regularly update regional stakeholder databases and maintain accurate contact records.
Team Collaboration & Continuous Improvement
  • Actively contribute to a supportive and collaborative team culture.
  • Participate in service reviews, team meetings, and initiatives aimed at enhancing practice and outcomes for First Nations clients.
Requirements
  • Identified position: Applicants must identify as Aboriginal and/or Torres Strait Islander.
  • Demonstrated understanding of the cultural, social, and systemic factors impacting First Nations communities.
  • Experience in case management, social work, or community services (preferred).
  • Strong interpersonal, documentation, and organizational skills.
  • Commitment to confidentiality, ethical practice, and client empowerment.
  • This position requires full-time, office-based attendance in the Brisbane CBD, five days per week. Remote or work-from-home arrangements are not available.
  • A valid driver’s licence is essential, as occasional travel may be required as part of the role.
