- Develop tailored case management plans addressing both immediate and long-term client needs using a strengths-based, person-centred approach.
- Conduct risk reviews, create safety plans, and support clients through referrals to specialist services including housing, AOD, and mental health.
- Assist clients in the transition to independence, supporting them with tenancy preparation, housing applications, and sustaining long-term accommodation.
- Maintain accurate and timely documentation, including case notes, reports, and data entry, ensuring compliance with Mission Australia’s case management standards.
- Build strong working relationships with clients, Indigenous communities, government and non-government agencies, and other co-located service providers.
- Facilitate or contribute to engagement activities, inter-agency meetings, service promotion, and program quality improvement initiatives.
- Provide mentoring and coaching support to less experienced staff and contribute to team learning by sharing best practice approaches.
- Relevant qualifications (e.g., degree or diploma) with professional experience in case management and homelessness services, including a minimum of 2 years working with vulnerable clients.
- Proven ability to manage a caseload of clients with complex needs, demonstrating trauma-informed practice and a strengths-based approach.
- Strong cultural capability and experience working with Aboriginal communities, with knowledge of local cultural protocols and systems, and ability to build respectful and professional relationships.
- Strong written and verbal communication skills, including the ability to manage administrative tasks and maintain professional boundaries.
- Resilience and adaptability when working with clients experiencing distress, trauma, AOD use, and aggressive or uninhibited behaviours.
- Proficiency in Microsoft Office applications, including Outlook, Word, Excel, and PowerPoint.
- A commitment to person-centred approaches and a belief in individuals’ capacity for recovery and growth, informed by lived experience and professional development.
- First Aid Certificate.
