Key Responsibilities:
- Long Length of Stay (LOS) Meetings: Lead and participate in multidisciplinary meetings to review and manage patients with extended hospital stays, aiming to identify and address barriers to timely discharge.
- Key Performance Indicators (KPIs): Monitor and report on clinical performance metrics, implementing strategies to achieve and exceed established KPIs.
- Complex Discharge Planning: Develop and oversee comprehensive discharge plans for patients with complex needs, ensuring safe and effective transitions from hospital to home or other care settings.
- Family Meetings: Facilitate discussions with patients and their families to provide information, address concerns, and support decision-making regarding care and discharge plans.
- Interfacility Transfers: Coordinate the transfer of patients between healthcare facilities, ensuring all clinical and logistical aspects are managed effectively to maintain patient safety and care continuity.
- Registered with the Australian Health Practitioner Regulation Agency (AHPRA) as a Registered Nurse
- Clinical Nurse Consultant/Clinical Midwife Consultant Grade 2 Having at least 5 years full time equivalent post registration experience, with at least 3 years full time equivalent experience in the specialty field, preferably in an acute setting and experience in patient flow and a community setting. In addition, must have approved postgraduate nursing qualifications relevant to the field in which he/she is appointed, or such other qualifications or experience deemed appropriate by the public hospital or public health organisation. An employer may also require a higher qualification in the specialist nursing field where such a qualification is considered essential for the performance of the individual position.
- Demonstrated ability to apply clinical expertise in a consultant role and the ability to work independently.
- Proven clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
- Demonstrated evidence of the use of critical thinking as it relates to the delivery of advanced evidenced based nursing care.
- Demonstrated experience in leading and implementing change, and developing direction in clinical practice by initiation, utilisation, review and translation of evidence-based research into practice, within a person-centred framework.
- Demonstrated effective interpersonal skills, excellent written and verbal communication skills, advanced computer skills and use of software programs to analyse data and produce reports.
- Demonstrated ability to contribute to strategic and clinical service planning aiming at reduced hospital length of stay, alternate care pathways and experience in navigating complex discharges and family dynamics.
