We pride ourselves on assisting individuals, from a range of diverse cultural backgrounds, to live and participate in their community as they choose.We are relocating to the vibrant heart of Sutherland, within a newly constructed, state-of-the-art office building, close to public transport, food hubs and gyms!About The RoleAs a Sylvanvale Support Coordinator you will be trusted to proactively support the client in a diverse mix of supports that enhance their capacity to maintain relationships, manage service delivery tasks, and actively participate in their community.Key responsibilities include:
  • Work with clients and their stakeholders to enhance their understanding of the NDIS.
  • Organise service agreements and gather quotes for services.
  • Monitor participants' plans and collaborate on developing future goals to drive meaningful outcomes for them.
  • Plan and coordinate providers, services and supports that work together to help the client achieve their goals.
  • Connect clients with NDIS services and supports and facilitate the setup of service agreements.
  • Coordinate various stakeholders and liaise with government and non-government agencies.
  • Coach and build capacity of clients to develop the skills to manage their own plans effectively.
  • Report to the NDIA on plan effectiveness and budget management and make funding recommendations.
  • Recommend and provide supports and services tailored to best meet the client’s needs.

  • Relevant tertiary qualifications or working towards, or relevant experience.
  • Ability to multitask and work within deadlines and towards key performance targets.
  • Ability to analyse and evaluate information.
  • An understanding of working within the NDIS billable hours environment and KPI’s.
  • Strong interpersonal, written and verbal communication skills
  • Confidence in effectively communicating with a range of stakeholders.
  • Ability to manage conflict of interest and maintain professional boundaries effectively.
  • Commitment to ongoing professional development.
Desirable
  • Comprehensive understanding of the NDIS framework and processes.
  • Experience as a Support Coordinator is highly desirable.
  • Experience working with Key Performance Indicators.
Please note that an unrestricted NSW Drivers License, full work rights, a NDIS Worker Screening Check, a current Working with Children Check and a police clearance are mandatory requirements for this position.Benefits of working with us
