Join the Hunter Valley Transitional Aged Care Program (TACP) and make a real difference in the lives of older people as they regain their independence after a hospital stay. Our passionate, multi-disciplinary team of Allied Health and Nursing professionals work closely with clients and their families, focusing on enablement and providing tailored, slow-stream therapy that helps people improve their function, confidence, and safety at home.As part of our home-visiting service, you'll deliver meaningful care where it matters most, in the client's own environment, helping them achieve their goals and thrive in their daily lives. The service runs year-round, Monday to Friday, 8:00am-4:30pm, offering both variety and stability for our Allied Health staff.About the role:
We have an exciting opportunity for a Physiotherapist to deliver high-quality clinical care that makes a meaningful difference to clients and their families. In this role, you'll assess and support rehabilitation needs, providing consumer-centred and family-inclusive interventions that promote wellbeing, independence, and social empowerment within a recovery-focused framework.You'll be well supported in your role, with onsite training to build your skills and guidance from experienced Level 3 Physiotherapists, ensuring you have the resources and mentorship to thrive.Requirements:
  • Physiotherapist with current Authority to Practice with AHPRA.
