About the Role
You will engage and support consumers, carers and supporters to access mental health care and services for improved outcomes. You will be integrated within and supported by the community based clinical mental health teams providing an introduction to allied health and case management within mental health.
  • Work within the Strengths Model of recovery orientated practice.
  • Engage and support consumers and their families/carers navigating the range of services available and provide information about other community organisations
  • Work collaboratively within the multi-disciplinary team.
Classifications for this position will be Occupational Therapist Grade 1 or Social Worker Grade 1 YB16 - YB19 or YC35 - YC39 ($73,808 - $84,572 per annum)Your Contribution
  • Social Workers - An approved degree in Social Work and eligibility for membership of the Australian Association of Social Workers
  • Occupational Therapists - Registration with the Occupational Therapy Board of Australia
  • Current Victorian driver’s license
  • Current National criminal history check, or willing to obtain.

About Hawthorn Community Mental Health ClinicThe Continuing Care Team based at Hawthorn Clinic provides specialist treatment and support to meet the identified needs of consumers who have a lived experience of mental illness in the City of Boroondara.
