In addition, Mental Health Services are provided at Epping, Broadmeadows, Jacana, Preston, Mill Park and Coburg.About the Role:This role requires experienced Registered Nurse to join the Hospital Admission Risk Program (HARP) Care Coordination team at Northern Health, in a fixed term positionThis role is responsible for delivery of safe, high quality care to clients with complex needs and/or chronic disease who are at risk of readmission to hospital.To be successful in this role you will have:
  • Minimum 5 years clinical experience
  • Experience and excellent knowledge in care coordination or case management
  • Clinical Leadership in the continuing care setting
  • Demonstrated advanced clinical assessment, intervention planning and delivery skills.
  • Demonstrated high level of knowledge of community, residential and acute/subacute services, that support clients with chronic disease and complex needs.
  • Demonstrated high level problem solving and planning ability.
