- Develop and implement culturally appropriate, strengths-based case management plans tailored to individual client needs.
- Provide person-centred case management and conduct regular reviews to monitor progress against case plans.
- Advocate strongly for clients with Territory Housing and other stakeholders to support sustainable tenancy outcomes.
- Provide practical tenancy support, including assisting with property care, light cleaning, and facilitating rubbish removal when required.
- Maintain accurate client records, reports, and data entry in accordance with program and organisational requirements.
- Build and sustain collaborative relationships with external agencies, attending community meetings and networks to strengthen service delivery.
- Provide weekly emergency relief services, including needs assessments and provision of essential support items.
- Attend and actively participate in local network meetings to strengthen collaborative partnerships with government and non-government organisations.
- Strong understanding of Aboriginal cultural systems and protocols and the ability to deliver culturally safe and responsive services.
- Relevant qualification in Community Services, AOD, Mental Health, Housing & Tenancy, or Social Sciences (degree, diploma, or equivalent experience).
- Minimum 12 months’ experience delivering case management with clients presenting complex needs.
- Strong understanding of housing and tenancy issues, particularly those impacting people at risk of homelessness.
- Experience working respectfully with Aboriginal and Torres Strait Islander peoples, with knowledge of cultural systems and protocols.
- Proven ability to manage a high-volume caseload, prioritise competing demands, and maintain professional boundaries.
- Excellent organisational, written, and verbal communication skills, with proficiency in Microsoft Office and client record management systems.
- Ability to work both independently and collaboratively as part of a team, with a professional, client-focused approach.
- A valid Northern Territory Driver's Licence and the ability to provide outreach and transport support for clients.
