The Community Care Tuberculosis Care Coordinator is part of a Multidisciplinary Team with a LHD wide focus on Tuberculosis treatment, prevention and refugee health. As the Tuberculosis Care Coordinator, you will use best practice to evidence provide person-centered specialist nursing care at an advanced practice level for TB patients in Southern NSW. This includes ensuring consistency of practise for high risk patient assessment, care planning and referral. The role coordinates and supports the LHD TB clinics working in partnership with tertiary providers and General Practitioners ensuring a flexible quality TB management and treatment service.Additional Information
- This position is open to Registered Nurses with a minimum of 3 years experience orClinical Nurse Specialists Grade 1 who would like to work towards a Clinical Nurse Specialist Grade 2 position.
- It will be the successful applicant's responsibility to complete their BCG and TST qualification. The costs of completing this qualification is the applicant's responsibility.
- Scholarships may be available to support gaining relevant immunisation accreditation.
- Demonstrated competencies in and knowledge of the complexities and challenges involved in providing speciality nursing care in the context of a multidisciplinary team.
- Demonstrate ability to exercise extended autonomy in clinical decision making capabilities and leadership in the development of nursing specialty practice advancement.
- Demonstrated effective clinical communication through the use of information technology, written and verbal means and sound interpersonal skills with the capacity to interact with others in a personable and professional manner.
- Demonstrated knowledge and application of evidence based practice and critical thinking to advocate for and implement individualised person-centred care at an extended role level with a commitment to quality improvement.
- Demonstrated ability to supervise, delegate, and negotiate with others to deliver quality nursing care with a willingness to show leadership and embrace change in the coordination and delivery of care
- Evidence of participation in own professional development, a commitment to maintaining competence as an authorised nurse immuniser (holding current accreditation to administer tuberculin skin testing (TST) and Bacillus Calmette-Guerin (BCG) vaccinations) and enhancement of skills (Tuberculosis disease related) with a willingness to apply and share professional expertise as a resource person and mentor others within the speciality.
- Evidence of a current unrestricted drivers' licence and the ability to maintain. Ability and willingness to drive/travel as the role requires. This may involve driving long distances and overnight stays.
